AFTER BOOKING, NEW PATIENTS WILL BE EMAILED TWO FORMS -'GDPR form' and 'Osteopathic Consent' form . PLEASE COMPLETE BOTH OF THESE IMMEDIATELY TO MINIMISE ADMINISTRATION TIME ON THE DAY OF YOUR APPOINTMENT.
A THIRD FORM 'INFECTION TRIAGE' will be sent BY EMAIL 2 days before EACH appointment. PLEASE COMPLETE AS SOON AS YOU RECEIVE THIS TO GIVE ME TIME TO CHECK THROUGH.
If you are unable to keep your appointment, I require at least 24 hours notice otherwise a fee of £38.00 will be charged.
The practice is open on Wednesdays and Thursdays and some Saturday mornings.
A generous 35-40 minutes is allowed for each appointment which enables us to discuss your problem and for me to provide osteopathic treatment as well as explanation and advice.
